LivingConnected

For members of CCS Medical's Managed Care partners

The LivingConnected program is a cellular-enabled mobile diabetes management program designed to give diabetes patients access to live monitoring and intervention when they have an adverse reading or trend in their blood sugar levels.

The goal of LivingConnected is to help patients manage their diabetes in real-time, which helps prevent complications and improve their quality of life. Results of live readings can be shared with others, allowing immediate health coaching from Registered Nurses and Certified Diabetes Care & Education Specialists. Ongoing reports and education are also part of the coordination of the patient's care.

For employers and other health plan sponsors, CCS Medical's LivingConnected program offers the two key components necessary to begin properly managing risk around its employees and covered dependents who have been diagnosed with diabetes: visibility into the size and scope of the problem, as well as control in managing the at-risk workforce towards optimal health. ¡Vida Latina!

For members of CCS Medical's Managed Care partners

Because of the high incidence rate for diabetes among the Latino population (two to four times that of non-Latino Caucasians), CCS Medical has a special Latino program called "¡Vida Latina!" to address this group and the early onset of diabetes that often occurs.

Designed to be culturally relevant, "¡Vida Latina!" reaches out to the Spanish-speaking community through:

  • A Spanish Specialty Team that has a bilingual staff of 12, who come from five Latino countries
  • A dedicated Spanish toll-free number 1.800.842.6021
  • Certified Diabetes Educators who can assess patients, along with their backgrounds, including their cultural beliefs, influences and lifestyle.
  • Written correspondence in Spanish adapted to the dialect of the patient
  • "One-call Resolution" to assist patients with all their needs, including diabetes, pharmacy, insulin pump, and billing.
  • Numerous educational resources in Spanish, including a Diabetes Education Guide, diabetes nutrition placements and carb counting handouts.

CCS Connects

For members of CCS Medical's Managed Care partners

CCS Connects is the longest running diabetes education program of its kind and was recently named a Diabetes Education Accreditation Program (DEAP) by the American Association of Diabetes Educators (AADE).

CCS Connects deploys wireless glucose technology and clinical monitoring to help patients adhere to their plan of treatment. The program is administered by a clinical team of Registered Nurses, Registered Dietitians, Registered Pharmacists and Certified Diabetes Educators.

CCS Connects reduces diabetes-related emergency room visits and hospitalizations by providing direct assistance to patients through a variety of methods:

  • Health risk surveys
  • Educational resources
  • Product training and adherence programs
  • Clinical assessment and education
  • Nutritional counseling, coaching and goal setting
  • Community programs

The success of CCS Connects is evidenced by HEDIS and STAR related outcomes, including a 95% long-term member satisfaction rate and a 25% higher adherence by members in completing recommended exams.

LivingLinked is a program offering support and assistance to those diabetes patients who use an insulin pump and/or a Continuous Glucose Monitor (CGM). Patients receive personalized service from a team of Registered Nurses and Certified Diabetes Educators.

The goal of LivingLinked is to help these patients stay updated on the insulin pump and CGM technology and motivated to control their blood sugars, giving them more freedom and better health. Patients discover new tips and techniques to achieve better diabetes management, which can result in improved glucose control throughout their lives:

  • Tools for carb counting
  • Wearing and using an insulin pump
  • Choosing the best pump supplies
  • CGM use and insurance coverage
  • Online resources
  • Updates on nutrition
  • Advanced features for basal and bolus management
